Are you talking about the Belltech 6400 shackle?
On a ranger, it actually lifts the rear. Unless, yes like you said, flip the hanger and it will lower it. you might run into interference issues though with it being so tall.
Here is it installed on my truck. It is on the second hole up for about a 1inch lift:

The leafs are below the axle to give it a 5inch drop. then a lift shackle to raise it up 1inch, netting a 4inch drop.
The easiest way to level out the back is to flip the shackle. It is pretty cheap. that gives you about a 2inch drop.
Probably the best way is to do an axle flip kit like mine, then put a lift shackle in. that gives you a 3inch drop. Combine with a 2in drop coil in the front and you got a nice looking lowered truck.
