If you dont have a crossmember, I was looking at tommys truck and was going to just get a piece of 2x4 thick steel and weld it in between the frame rails with some gussets on the backside.

never pull someone out backwards.. well at least if you have a 4x4 dont its bad on the transfer case...
get a rear hitch with a shackle receiver...
a front hitch is tons stonger then those tow hooks. for one you have to pull from BOTH at the same time or risk tweaking your front cross member and or frame...

SEE thats the problem... dont look at it that way lol... just get **** as it comes.. hell i have spent over oh god 10k easy on mine if not more...
also keep and eye out for rear one's on like craigs list thats if you dont have one.. hell i got mine free it looked like crap i had it sand blasted and it looks great now lol

the front frame on your 04 is the same as the 4x4 frame, the mounting holes are there and factory tow hooks will bolt right in. ford would not put them there if it was not safe to use them, forward or reverse.
my 04 did not come with hooks, it "WAS" 4x2 when i bought it. i simply ordered new ones from ford and bolted them in place.
this pic is after i moded the frame to get them back in place after the body lift
