2002 XLT 4X4 4.0 5-speed manual - CLICK/TICK noise (VIDEO)

Truck has 129,500 miles. The sound increases with rpm's. Any ideas what this could be?

The noise is timed perfectly with the timing belt movement.
Did you look to see if anything is hitting in the belt area ?

Nothing is hitting that I can see. I thought maybe the belt tensioner, but that usually sounds different from what I've seen on the web.
